LLC and the New Jersey Center for Healthy Living are

you secure and confidential. The following is information
to help you understand what this means and how we work
to fulfill this commitment:
In the course of our health care services to clients and
patients, we may collect a variety of personal information
about you including:
Information we receive from you when you place your name
on our mailing lists.
Information we receive from you on service intake or other
forms or via telephone conversations with you.
Information about your transactions with us such as purchases
and credit card transactions.
The transactions and health care services that you participate
in at the NJ Center for Health Living are a matter of personal
choice as well as a private matter between you and your
doctor/therapist. All of your health care records are private.
We will not release any records that are on file at the
Alliance for Integrated Healthcare and/or NJ Center for
Healthy Living facilities without your written permission.
Doctors maintaining their practice at the NJ Center for
Healthy Living comply with HIPAA requirements. You will
be required to sign a HIPAA form during the first visit
with your doctor.
What does “HIPAA” stand for? HIPAA is an acronym
for the federal law entitled “Health Insurance Portability
and Accountability Act” of 1996. The privacy provisions
of this law apply to health information created or maintained
by health care providers who engage in certain electronic
transactions, health plans, and health care clearinghouses.
There is a regulation issued by the Department of Health
and Human Services entitled, "Standards for Privacy
of Individually Identifiable Health Information".
This regulation applies to all entities covered by HIPAA.
The Office for Civil Rights is the department of federal
government that is responsible for implementing and enforcing
the privacy regulation.
